Appointments
If you would like to make an appointment, please contact our office by phone or email. Or, if you would like, feel free to use our online appointment feature, download the New Patient Forms, and we'll confirm your appointment within 24 hours.
Financial
We will be more than happy to verify your insurance benefits for you. Once those benefits have been obtained, we will reserve time to explain in detail the benefits afforded you by your particular insurance carrier. We submit all insurance forms for you, and offer numerous payment options, allowing you to you afford the corrective treatment you need. Forms of payment accepted by our office include: Family Plans, Monthly Payment Plans, major Credit Cards, and the Acclaim! Card. Even if you don't have insurance, or have a high-deductible plan, don't worry- we can help. After all... if you can't afford us, we can't help you!
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A $25 fee will be charged for such.
